Letter Request Form

If you wish to request a letter from OIS to invite family or to confirm visa status, please complete this form. It is designed for current University of Maryland F-1, J-1, and H1-B visa holders; however, exceptions may be made for other visa categories on an as needed basis, including Permanent Residents.

Letters can be picked up 5 business days after the request at 2111 Holzapfel Hall, College Park, MD 20742.

B. Type of Letter

DIRECTIONS: There are six different categories: transfer, health insurance, full-time visa status, social security number recommendation, general invitation, and graduation invitation. Please select one of them. If these letters do not meet your need, select other type at the bottom. Special letters may require additional processing time.

Confirmation of Full-time Visa Status
If you need "Full-time registration confirmation", the Records & Registration Office on the 1st floor of the Mitchell Building will issue it with proof of your student ID number.

Social Security Letter (New students must attend a Documents Check session before request)

Note: Social Security letter requests are available for F-1 and J-1 visa holders. When applying for an SSN at the Social Security Administration, you will need to take this letter along with original copies of your offer letter and a "letter of employment" that indicates that you have ACCEPTED the offer of employment OR a pay stub, as well as your immigration documents, including your I-94 card, I-20 or DS-2019, and a passport that has been valid for at least one year.
